Elective Total Hip Arthroplasty (THA) Cohort
The elective THA cohort is defined by a principal procedure of Total Hip Arthroplasty, excluding a number of other conditions and procedures that would indicate the procedure is not elective or that increase the likelihood of complications. This is aligned with the CMS Procedure-Specific Readmission Measure for elective THA/TKA.
To leverage this cohort:
- Profile filters: In the Profile Editor, add the cohort as a profile filter. With this option, you can further customize the profile with additional filters.
- Statistical Process Control: Add a Statistical Process Control section to a Scorecard and select the cohort from the dropdown. Within this section, the cohort is not customizable and must be used as-is. When drilling into a chart in the SPC section, you can select to create a profile of the encounters you're drilling into so you can analyze and customize further.
Cohort inclusions
To be eligible for the THA Cohort, the encounter must have an ICD-10 Principal Procedure Code for a hip replacement (one of: 0SR9019, 0SR901A, 0SR901Z, 0SR9029, 0SR902A, 0SR902Z, 0SR9039, 0SR903Z, 0SR9049, 0SR904A, 0SR904Z, 0SR90J9, 0SR90JA, 0SR90JZ, 0SRB019, 0SRB01A, 0SRB01Z, 0SRB029, 0SRB02A, 0SRB02Z, 0SRB039, 0SRB03A, 0SRB03Z, 0SRB049, 0SRB04A, 0SRB04Z, 0SRB0J9, 0SRB0JA, or 0SRB0JZ).
Cohort exclusions
Encounters with any of the following conditions are excluded from the cohort:
- Any diagnosis code for pelvic fracture or fracture of lower limbs
- Any procedure code for partial hip arthroplasty
- Any procedure for revision, resurfacing, or implanted device/prosthesis removal
- Principal diagnosis is for mechanical complication
- Principal diagnosis is for malignant neoplasm of pelvis, sacrum, coccyx, lower limbs, or bone/marrow or a disseminated malignant neoplasm
- Admission source or discharge disposition indicates transfer
- More than 3 procedure codes for THA or TKA are present

Statistical Process Control (SPC) measures

- 1 to 30 Day Readmission Rate Forward
- Charges - Principal Procedure (sum of OR Charges on the day of the Principal Procedure)
Adverse events
These measures flag the encounter if the condition is present. The adverse events are aggregated in the Adverse Event bar chart.
- Any PSI
- Any HAC
- Usage - ICU
- Usage - Blood Use
- AMI within 7 days of index admission
- Pneumonia within 7 days of index admission
- Sepsis within 7 days of index admission
- Surgical site bleed within 30 days of index admission
- Pulmonary embolism within 30 days of index admission
- Mortality within 30 days of admission
- Mechanical complications within 90 days of index admission
- Periprosthetic joint/wound infection with 90 days of index admission
- Unexplained Cardiac Arrest
- Reaction to Anesthesia
